Through a new, innovative partnership with Nova Scotia Health (NSH), we are opening the first transitional care center in our province, in West Bedford. Shannex is the operating partner for the transitional care centre on behalf of Nova Scotia Health. you will have an opportunity to work in a facility with a purpose and design dedicated to providing care to NSH patients in central zone, who no longer require hospital care, but need services before they can safely return home/community. We are searching for a Registered Nurse to join our Transitional Care Centre team based in West Bedford, Nova Scotia. The transitional care model supports patients/families who require additional time, services, and support to transition back to home/community, by providing an alternate space with intentional programming, building design, and resource allocation to increase the probability of returning to their previous living situation. (e.g. decrease in appetite). - Ensures all completed care tasks are documented appropriately using identified technology and tools. - Ensures Continuing Care Assistants/PSWs are completing care in a timely manner.
