<p>Mission Statement: Homes First Society develops and provides affordable, stable housing and support services to break the cycle of homelessness for people with the fewest housing options.</p><p><strong> </strong></p><p><strong>Location: Scarborough (near Kennedy Rd & Eglinton Ave E)</strong></p><p><strong>Hours of Work:</strong><strong> Monday to Friday, 9am to 5pm, with one evening per week</strong></p><p><strong>Contract Period: starting ASAP, ending 15 November 2024</strong></p><p><strong>Salary: $28.76 per hour, plus 7% in lieu of benefits</strong></p><p><strong>Submit Resume to: ramya.rengaraman@homesfirst.on.ca</strong></p><p><strong> </strong></p><p></p><p><strong>Applicants must quote ?CICM-24? </strong></p><p></p><p>This position supports clients to access housing and other support services while in the shelter system, while adhering to the mission statement and policies and procedures of Homes First Society.</p><p></p><p>Job Description:</p><ul><li>Provide wrap around case management to clients on a dedicated caseload to support them to access housing by supporting clients to get their identification, complete taxes, access a source of income, etc</li><li>Work with clients to complete housing applications to access housing options such as social housing and supportive housing options</li><li>Make referrals to the Housing Help Worker to support the client to access housing</li><li>Connect clients to follow-up supports once housed to ensure they stabilize into that housing dependent on client?s level of needs</li><li>Ensure that client information collected is shared with co-workers on the client services team to assist in determining the clients? needs and developing a client case plan</li><li>Attend client services team meetings to present client cases and discuss carrying out support plans with clients on the caseload</li><li>Complete and utilize assessment tools to gather information to inform the development of client case plans</li><li>Develop, implement and monitor individual client case plans and crisis plans for each client on a client services caseload</li><li>Respond to conflicts, utilizing crisis intervention, and desecration, and prevention skills when required</li><li>Develop working relationships with clients to provide wrap around supports and access to housing</li><li>Accompany clients to appointments based on their individual case management plans</li><li>Work with other Intensive Case Management Workers to develop and provide referrals for specialized supports to clients based on client?s goals, needs, and choices</li><li>Work from a Housing First, Harm Reduction, and Trauma informed care philosophy</li><li>Within your team, regularly visit clients and update assessments, case plans, and consents to support client housing stabilization</li><li>The team will be familiar with Canada Revenue Agency, Service Canada, Service Ontario processes in order to support clients to access a source of income</li><li>Regularly complete client documentation, reports on clients, and providing data to complete reports</li><li>Other duties as required</li></ul><p></p><p>Qualifications:</p><ul><li>Minimum one year experience in related post-secondary education, student placements, or equivalent combination of education and experience</li><li>Ability to compile information to complete reports</li><li>Demonstrated ability to utilize case management software systems</li><li>Highly development case management skills</li><li>Experience with conflict resolution and negotiation skills</li><li>Demonstrated ability to connect clients to Ontario Works, Office of the Ontario Public Guardian and Trusteeship, Ontario Disability Support Program, and the process for accessing The Housing Stabilization Fund</li><li>Knowledge of mental health and addiction issues and several years? experience working with clients</li></ul><p>with these issues</p><ul><li>Experience with individuals who lack primary health care and/or struggle to mainta
