Working closely with the entire Support Services Department, Case Managers provide both in-house and outreach-based support to people in our community with a variety of needs. Case Managers assist individuals in accessing resources in the community such as housing, healthcare, and justice support. -Act as an advocate for people who access services (PWAS)-Conduct yourself in a manner that maintains and promotes a positive image for Prairie Harm Reduction, and is in keeping with the policies, mission, goals, and philosophies of the organization-Work collaboratively with other programs in the building to ensure that programs are operating-Offer naloxone training to PWAS (train and distribute kits as needed)-Provide ongoing support, information, advocacy, referrals and system navigation to PWAS-Provide transportation for PWAS as part of their case plan-Maintain high knowledge of income assistance policies-Help PWAS navigate various systems (ie. justice, healthcare, human rights)-Maintain a high knowledge of HIV, Hepatitis C, harm reduction, and trauma-informed care-Maintain adequate knowledge and skills to ensure PWAS receives a high standard of services-Maintain detailed statistics in the communications log for case management and reporting including collating monthly statistics-Help with Christmas Hampers Program-Help plan and implement PHRs various fundraisers-Work collaboratively with community partners-Attend community events as directed-Attend training and professional development as instructed by the Director of Support Services-Other duties as assigned
