*Status:* 1.0 FTE (75 hours/bi-weekly)
*Hours of Operation*: Monday-Friday; Hours fall between 8:45am-8:00pm. Occasional weekends shifts may be required.

*JOB OVERVIEW*
The Manitoba HIV Program?s ?Program to Access Treatment for HIV and Support (PATHS)? is an interdisciplinary outreach based service delivery model that provides intensive case management for people living with HIV (PLHIV) in Manitoba who are not linked, or precariously linked to HIV care. The goals of the clinical outreach team are to:
· Provide wrap-around care with psychosocial supports
· Offer and link to Indigenous led and culturally safe care
· Support PLHIV to access and adhere to HIV treatment medication, and
· Support PLHIV to transition to long term primary care services
Reporting to AHWC Director of Primary Care, the PATHS Primary Care Social Worker works as part of an interdisciplinary team to provide clinical outreach services. The Social Worker will practice autonomy and will formulate clinical decisions and development of a patient care plan in collaboration with the multidisciplinary AHWC Clinics. Drawing from the strengths of this diverse team and from AHWC?s values and mission statement, this role requires a welcoming, kind, and compassionate person for community members who visit the PATHS Clinic to access supports during their wellness journey.
The Social Worker should demonstrate an understanding of and respect for the histories, traditions, and cultures of Indigenous peoples, inclusive of First Nation, Métis, and Inuit Communities and Nations, including an understanding of the importance of decolonizing healthcare systems and spaces.
*EDUCATION*
· Baccalaureate degree in Social Work from a recognized university
· Registered with the Manitoba College of Social Workers
· Addiction related training (such as The Core Addictions Practices certificate or equivalent education and training experience in substance use / dependency), an asset
*ESSENTIAL SKILLS *
· Four years related community experience
· Broad knowledge of systems theory and application to social work practice.
· Working knowledge of counselling skills and theories.
· Knowledge of Crisis Intervention Theory
· Effective Crisis Intervention Skills
· Expertise in the area of mental health and addictions
· Knowledge and understanding of psychosocial rehab and recovery
· Knowledge and experience in working with people with substance related concerns
· Proficient at case management
· Excellent communication and interpersonal skills
· Effective networking and agency relations skills
· Applied Suicide Intervention Skills Training (ASIST)
· Suicide Assessment and Intervention Training
· Trauma Informed Care Training
· Completion of MICST (Manitoba Indigenous Cultural Safety Training)
· Working knowledge of electronic medical records (EMR) preferred
*LICENSES, REGISTRATIONS*
· Responsible for maintaining and providing proof of active registration with the appropriate registering body
*PHYSICAL AND EMOTIONAL DEMANDS OF THE JOB*
· Physical ability to perform the duties, responsibilities, and functions of the job
· Must be able to function effectively in a fast-moving environment with frequent interruptions
· Should have good coping skills and stress management
· Requires mobility in community settings and at the worksite
· Requires good manual dexterity, visual and aural acuity
