*About Us:*
Reconnect Community Health Services is a not-for-profit health service organization in the west end of Toronto. Our services include supports for seniors, caregivers, and people living with mental health and addictions concerns. We pride ourselves on being at the forefront of client-centred care in our sector.
The Case Manager will work in partnership with Mid-West Sub Region Primary Care Inter-Professional team to assess, connect and provide case management to all clients referred to this service. The primary goal of the service is to work with the other team partners which includes hospitals and Community Health Centers, to ensure connectivity and support to primary care referrals who require support and to increase knowledge and connections for primary care providers with community-based supports.
*Responsibilities* include, but are not limited to, the following. Other duties may be assigned at the discretion of the Program Manager.
* Perform intake assessments and conduct ongoing needs assessments;
* Connect and build relationships with primary care providers and other stakeholders to increase knowledge of community resources and to encourage referrals to mental health services.
* Develop and implement a plan of care with short and long-term objectives, with ongoing monitoring until clients can be successfully discharged from service;
* Provide case management services which include assessment, intensive case management, long term planning, and follow up services when needed;
* Screen and respond to crisis calls initiated by clients, family members, friends, health care providers, and community agencies;
* Refer and link clients and caregivers to a range of community resources, including housing, legal supports, health supports, meals, drop-in centres, social recreational program, clothing exchanges, etc.;
* Monitor clients to supervise care, identify additional needs, and mitigate risks;
* Carry out case management and service delivery/client contact documentation requirement in a timely and professional manner;
* Collaborate with staff from the other organizations in the Inter-Professional Team to ensure that services to all clients are timely, flexible and coordinated
* Collaborates with judicial and other systems, organizations and individuals to ensure a well-coordinated service system;
* Liaise and collaborate with external services including: transitional care, acute care, emergency departments, rehabilitation centres, and other professionals, agencies and groups, to identify problems and possible solutions for service provision and improvement.
*Technical and Behavioral Competencies Required: *
* Has a university degree and/or Community College Diploma in social services or related field - BSW or MSW, preferred
* 3 Years related experience in direct service delivery to adults with serious mental illness
* Demonstrated experience working with primary care providers and stakeholders in building referral relationships and coordinating healthcare
* 1-2 years community development experience, with a focus on healthcare
* Experience and knowledge in crisis intervention and prevention
