Vitality Health Management is a leading primary care clinic in Toronto, Ontario, dedicated to delivering exceptional, patient-centered healthcare. We are seeking a compassionate and organized *Patient Care Liaison/Coordinator* to join our team. This role is vital in ensuring smooth clinic operations and providing outstanding support to both patients and providers.
*Key Responsibilities:*
* Track and manage doctors? orders to maintain efficient clinic operations and ensure timely patient care.
* Assist patients in understanding and implementing their personalized healthcare plans.
* Communicate with pharmacies and wholesalers to ensure timely procurement of medications and clinic supplies.
* Oversee inventory management, ensuring the clinic is well-stocked and organized.
* Support doctors with patient care and administrative tasks as needed.
* Act as the primary point of contact for patients, addressing inquiries, resolving concerns, and ensuring a seamless care experience.
* Manage client bookings and maintain accurate, up-to-date records in the electronic medical records (EMR) system.
*Qualifications:*
* *RPN or RN license is an asset* but not required.
* Experience in a healthcare or clinical setting is preferred.
* Strong organizational and time-management skills, with the ability to multitask effectively.
* Excellent verbal and written communication skills.
* Proficiency in using electronic medical records (EMRs) and standard computer applications.
* Knowledge of medical supplies, inventory management, and medication ordering processes.
* Fluency in Mandarin is an asset but not required.
