Key responsibilities include:
? Providing comprehensive patient-centered care across the lifespan, including health assessments, diagnostics, and treatment plans.
? Collaborate with a multidisciplinary team to deliver coordinated care for chronic disease management and acute health issues.
? Educate patients and families on health promotion and disease prevention.
? Contribute to quality improvement initiatives within the Primary Care Network.
Our ideal candidate has:
? A master?s degree in nursing (NP stream) with good standing with the College of Registered Nurses of Alberta (CRNA).
? Active registration and NP license with the CRNA. Evidence of renewal annually.
? A minimum of two years of experience in primary care, community health, or a similar setting (new graduates with a passion for primary care are welcome to apply!)
? Experience in using electronic medical records is an asset.
? Excellent relationship building, communication and interpersonal skills, with a commitment to patient-centered care.
This position reports to the Executive Director of the Drayton Valley Primary Care Network.
Job Types: Full-time, Permanent
Pay: $125,000.00-$145,000.00 per year
