Position #: 152733 Expected Start Date: December 27, 2024 Union: SUN Facility: Hafford Hospital and Special Care Home City/Town: Hafford Department: Primary Care Sites Type: Full-time regular FTE: 1 Hours of Work: 112 hours per 3 weeks rotation Relief: No Float: No Field Hours: Yes Salary or Pay Band: Pay Band Nurse Practitioner $52.590 to $62.910 Travel Required: Yes Job Description: In Collaboration with Physicians and the Primary Care Team, the Nurse Practitioner will Provide Primary Health Care Services to Several Communities. This Includes: Health Promotion and Protection, Disease and Injury Prevention, Provision of Supportive, Instructive, Treatment and Emergency Services; Collaboration with and Referral to other Health Care Professionals; Conducting Community Needs Assessments and Participating in Evaluative Projects. The Incumbent Reports to The Director of Primary Care for the Prince Albert Parkland Health Region.
