Vancouver Coastal Health is looking for a Social Worker to join the Inner City Intensive Case Management Team in Heatley Community Health Centre in Vancouver, BC. Apply today to join our team! As a Social Worker with the Vancouver Coastal Health, you will: Provide advanced providing clinical and consultative social work services to clients and families. Conduct psychosocial assessments. Provide individual, family and group counselling services. Maintain client records. Attend interdisciplinary clinical/case conferences. Identify applicable community resources for clients and facilitating care throughout the continuum. Qualifications:Education & Experience Current full registration with the British Columbia College of Social Workers- LocationVancouver, BC
