Support and advocacy to clients and elders in the community. SUPPORTS NEEDED Provides individual home care, advocacy, communication, health and wellness programs to elders, persons with disabilities and others in the community with identified need Provides home care including general housekeeping, personal hygiene and provides simple meals or snacks as scheduled Develops communication tools and posters to enhance participation in health and wellness programs or services Assists with exercises, physiotherapy and other medical tasks assigned by the Home Care Nurse or Supervisor. Develop and deliver programming to provide clients and elders exercises, health education and socialization, facilitate when required Follows medical instructions carefully and consistently, adheres to Delegation of tasks as directed by the health professional. Including monitoring vitals and conditions Organizes time and resources based on the individual needs of home care client or elder Manages any unanticipated events as directed by the supervisor and within scope of practice Delivers the Programs and services in an appropriate, caring and respectful manner Identifies persons requiring home care and directs individuals and their families to appropriate programs referrals and advocacy Coordinates appropriate care and equipment including other community resources as required Consults with family members and other supports to ensure that care in on-going and that all client needs are identified and met Encourages clients and families to be involved in care take responsibility for care Liaises with all family, medical and other resources Advocates on behalf of clients and community elders for additional services and resources All persons employed by the Okanagan Indian Band may be required to assist the Band in providing emergency services. Duties assigned during an emergency may differ from regular duties Participates in community events as requestedADMINISTRATIVE DUTIES Communication with clients, elders, internal and external resources as required Records home care services in confidential client file after each visit, provide monthly reports on duties performed outside of direct care Educates clients, families and support systems about the Home Care Program and Elder programs available, informing each about criteria, costs, registrations and process Reports any concerns to the supervisor. (Risks, incidents, changes in condition et.) Attends meetings collaborating with programs and departments as required to enhance services for clients and elders. Provide advocacy where required Ensures adherence to Home and Community Care Policy Manual, the Contribution Agreement from Health Canada, Indigenous Services Canada policies and procedures Performs all duties and responsibilities in accordance with the OKIB policies and proce
